Sometimes referred to as “Swing Bed” care, Transitional Care bridges the gap between hospital care and returning home by providing skilled nursing care, rehabilitation services, and individualized treatment plans in a comfortable, supportive environment.

What is Transitional Care?
Transitional Care is a short-term skilled care program designed for patients who no longer require acute hospital treatment but still need medical monitoring, rehabilitation, or nursing support before safely returning home.
Patients may benefit from Transitional Care following:
- Joint replacement surgery
- Stroke recovery
- Fractures or orthopedic injuries
- Cardiac conditions
- Pneumonia or respiratory illness
- General weakness or deconditioning
- Extended hospitalization
