Cardiac Rehabilitation

Cardiac Rehabilitation at Girard Medical Center is designed for people recovering from heart-related conditions or procedures. The program supports patients through supervised exercise, education, and long-term heart-healthy lifestyle planning. Girard Medical Center staff will tailor your program to your specific needs.

Patient Cardiac Rehab: The Girard Medical Center Phases to Lifelong Health

Girard Medical Center’s Cardiac Rehabilitation program includes two phases: Phase II outpatient rehab and Phase III long-term maintenance. Phase II begins as soon as you leave the hospital and begin the outpatient program. Phase III is the long-term maintenance and intended to instill lifestyle practices that will become lifelong habits.

For Cardiac Rehabilitation Patient Referrals

Ask your provider to complete the referral form to get started.

Referrals are required for the program. Ask your medical professional for a referral if you meet any of the following requirements:

  • Recent heart attack within 12 months
  • Coronary stenting
  • Stable angina or stable CHF
  • Coronary artery bypass graft
  • Valve surgery
  • Heart transplant

Pulmonary Rehabilitation

Pulmonary Rehabilitation is a specialized outpatient program for patients living with lung disease and breathing challenges. Our team uses a multidisciplinary approach that focuses on exercise conditioning, strength training, breathing retraining, education, emotional support, and strategies for managing daily activities.

Who may benefit from Pulmonary Rehab?

Pulmonary rehab may be appropriate for patients with:

  • COPD
  • Asthma
  • Emphysema
  • Interstitial lung disease
  • Pulmonary fibrosis
  • COVID-19 recovery
  • Other lung diseases and breathing challenges

What to Expect

This program is designed to help patients:

  • Improve endurance and muscle strength
  • Reduce pulmonary events
  • Learn proper breathing techniques
  • Use pacing and energy conservation skills
  • Improve body mechanics for everyday tasks
  • Take a more active role in their health and recovery

Peripheral Artery Disease (PAD) Program

Girard Medical Center’s Peripheral Artery Program helps patients living with Peripheral Artery Disease (PAD) improve walking tolerance, manage symptoms, and better understand their condition through education and guided rehabilitation.

What is PAD?

Peripheral Artery Disease happens when arteries in the legs become narrowed or blocked by plaque buildup. This can reduce blood flow to the legs and feet, which may lead to discomfort, numbness, and a higher risk of complications.

How the Girard Medical Center PAD Program Supports Patients

Our PAD rehabilitation approach centers on a graded exercise program that gradually increases activity over time to improve tolerance for walking and exercise. Patients also receive education about PAD and ways to reduce risk factors.

Common Symptoms of PAD

  • Pain or cramping in the calves, thighs, or buttocks after walking
  • Numbness or tingling in arms, legs, hands, or feet
  • Weakness
  • Cold limbs
  • Changes in limb color

  • Current or former tobacco use
  • Diabetes or Hypertension
  • Personal or family history of heart disease
  • Heart attack or stroke
  • High cholesterol

  • Signed referral form
  • Patient demographics / face sheet
  • Qualifying diagnosis
  • PFT results, labs, medical history, latest progress notes, and medication list
  • Insurance information
